Massachusetts law requires college students to have certain immunizations before coming to school.

Learn about required immunizations and download the Massachusetts Required Immunization History form at www.umass.edu/uhs/immunizations. Follow all instructions carefully.

Forms must be submitted to the New Student Immunization Program (NSIP) office at UHS before the end of the Add/Drop period; failure to do so will result in a registration hold. For more information, call the NSIP, (413) 577-5275.
